As Independent Facilitators, we guide you through every step of transitioning into the Self-Determination Program:

1. Develop Your Person-Centered Plan (PCP):

   • Collaborate to create a personalized plan reflecting your unique needs and goals.

2. NOTE: Previously completed PCPs

   • Discuss the next steps to continue your transition

3. Review Your Regional Center Budget:

   • Understand and optimize your current budget within the Self-Determination framework.

4. Create a Customized Spending Plan:

   • Align your financial resources with your aspirations.

   • Vendor Collaboration: Work with existing or new vendors that best fit your needs.

   • Guidance on Selecting a Financial Management Service (FMS): Choose the FMS provider that suits you best.

These services are fully funded by your Regional Center, allowing you to start your journey at no cost to you."

After enrollment, as Independent Facilitators, we offer ongoing support to ensure your plan remains aligned with your evolving needs:

• Regular Updates to Your PCP:

  • Adjust your plan as your goals and circumstances change.

• Monitoring Your Spending Plan:

  • Stay on track with your budget, making necessary adjustments.

• Collaboration with FMS and Service Coordinators:

  • Ensure seamless service delivery and coordination.

• Specialized Support and Advocacy:

  • Receive assistance with challenges and advocacy on your behalf.

These ongoing services can be included in your Self-Determination budget, ensuring continuous, personalized support."
